Hope this helps. When I first ran through Krokotopia with my Pyro, I thought the world was broken since I was fizzling so much. I started playing expecting to fizzle. I realize now that Krokotopia was where I started using my Firecat and Meteor spells more than I was using my wand. Eventually I started wearing gear that increased my accuracy.
Much nicer! Every once in a while though, when I fizzle three Heliphants in a row, I'll exclaim "What? Am I back in Krokotopia? I think there is a unusually high fizzle rate atm anyway. So, have you ever noticed that, besides the smoke coming out of the casting symbol, there are subtle tells to show ahead of time whether or not your spell fizzled? For example, if you notice, when you cast a fire spell, there will be two fireballs swirling around you, which are a dark, hearty red.
But if you fizzle, you will notice that these fireballs are a lighter shade of red than they would be if you hit? As well as for death spells, there is a cool circle in the middle of the skull symbol. But if you fizzle, that smooth circle will appear very grainy!

I have never noticed this, but if what you say is true, then maybe accuracy is not added but multiplied. It could also be a boost in how much chance of getting a fizz is removed i. The Queen of Ice. I know some bosses cheat, so they could remove your accuracy no matter if you do have Elixir, or spells for it or not. Lucas Rain. Too many people experience this kind of fizzling as an unexpected and bad surprise.
